Procedure for Expansion of Service Area for a Health Maintenance Organization

Service areas for a health maintenance organization (HMO) are issued on a parish by parish basis.   To expand the service area of a licensee you must submit the following:

• A written request from an officer of the HMO requesting expansion of the service area.
• A list of all parishes into which the licensee wishes to expand.
• For each parish provide a list of the healthcare providers in that parish. The list must be broken down based on specialty.
• An explanation as to how the HMO complies with the provisions of La. R.S. 22:1019.2
