Surrender of Third Party Administrator and/or Pharmacy Services Administrative License
A third party administrator and/or pharmacy services administrative organization license does not expire in Louisiana. Once issued the license remains in effect unless surrendered, suspended or revoked. The annual report filing and fee is not a renewal filing and failure to file the report and the fee will not result in the expiration of the license but may lead to other action.
To surrender a third party administrator and/or pharmacy services administrative organization license in Louisiana the licensee must provide written notice of the desire to do so and return the original license issued by the LDI or an affidavit stating that the original license has been lost. The notice must be signed by an officer of the licensee.

Prior to surrendering a TPA license, the licensee must file an annual report as of the previous December 31 and must also file a final report for activity since January 1 of the current year.
