Change of Address / Contact Notification

To submit a change of address or contact you will need to file a Uniform Certificate of Authority Corporate Amendment. There are no Louisiana-specific items and no fees are required.
Please note: if the address being changed is the domicile address and the articles of incorporation were amended in order to affect this change, the company must:
Electronic Submission of Application
If the application is submitted electronically using the National Association of Insurance Commissioners Uniform Certificate of Authority electronic submission process or via email to [email protected] there are no “hard copy” filing requirements for the application. However, on all imaged documents that require notarization or certification, the notarization or certification seal must be clearly visible on the image.
Alternative Filing Options
For changes in contact information only, insurers may consider using the Company Contacts module of the Industry Access Portal. The module allows real time access to view and modify the contact information on record with the LDI for an insurer and other regulated entities. If changes to contacts are made using the Industry Access Portal, there is no need to file the Corporate Amendment Application.
